And more importantly, can you actually reverse it? Today I am going to walk you through exactly why hair loses its color, what is happening inside the follicle while it happens, and what you can do in the comfort of your own home to slow this process down, stop it in its tracks, or in some cases reverse it, so that you can keep long, luscious, healthy colored hair for the long term. Gray hair is something almost all of us are bothered about, and it is an unfortunate fact of life that most people, as they grow older, are going to develop some gray hair. But some people develop it a lot earlier than others. So in this video I want to answer two questions. First, why do people get gray hair in the first place? Why is it even a thing? And second, can it be stopped, and can it be reversed? The answer might surprise you. Before we go into the deep dive, let me give you the quick answer up front. Yes, it can be stopped, and yes, it can be reversed. Keep watching, because the why and the how are the important parts. [ON SCREEN: "Grey hair is not grey"] So, first up, why do we get gray hair? Well, let us start with what gives hair the color it actually has, because gray hair is not really gray hair, and white hair is not really white hair. It is transparent hair. It is a lack of pigment that leads to that color. When light shines through it, it looks gray, it looks white, and that is why we call it gray hair or white hair. But really, it is colorless hair. A lack of pigment is what is actually going on. Now, in terms of pigment, our body's natural pigment is something called melanin, and we have melanin all over the place. You can see my skin is brown. That is because of melanin sitting in the skin cells. Hair is no different. The amount of melanin in the hair is going to determine what color that hair actually is. [ON SCREEN: "Meet the melanocyte"] I will put a picture up on the screen of the anatomy of a hair follicle. You will notice that at the bottom of the shaft there is what we call the bulge, which is a great name for that area. Inside that bulge you have what is called the bulb, the bulb of the hair follicle. And right next to it is where the melanocytes live. Melanocytes, if we break the word down, melano meaning melanin, cytes meaning cells. So melanocytes are melanin producing cells. These cells live right next to the bulb of the hair follicle, and as you grow older and as you go through life, they continually pump melanin into the bulb of the hair, which is where the hair grows from. That way all of the new hair is saturated in melanin and has color. That is why you have black hair, or brown hair, and so on. [ON SCREEN: "Then ageing happens"] However, there is a natural process we all go through called aging, and unfortunately it is something we cannot really stop. Remember that every cell has a certain life cycle. A blood cell, for example, only stays alive for about 120 days. That is why, if you are taking iron tablets in order to raise your iron levels, it will take three or four months before you see the actual iron level rise, because all the old blood cells need to die off first before the iron can be deposited into new blood cells. It is also why, when we are doing iron testing, we test every three or four months instead of every single week. Testing every week is pointless. As you age, cells divide, and that is how they make more cells. You have one single cell, it copies itself over into another cell. Every time you copy a cell over, you have to copy every single part of that cell, and the most important part is going to be the DNA, your DNA, which sits inside that cell. [ON SCREEN: "DNA = a very long code"] Some of you may know this and some of you may not, but DNA is super complicated. Essentially it is like a really long computer code of how to build our bodies. When you copy over that code, similar to copy and pasting on a computer, sometimes errors occur. You might copy a whole paragraph and one of the letters, or one of the words, comes out slightly wrong. This happens with DNA as well. You can copy the whole thing over into a new cell, but small errors do come up. A slightly switched letter here, a slightly wrong sequence there, and over time that leads to mistakes, errors, and damage to DNA. That is really what aging is. That is the process of aging, and that is what leads to so many age related diseases. Now, when you are copying these errors over, your body does have natural mechanisms to stop it. It has a scanner of sorts, similar to a spell check on Microsoft Word, where it goes through the text, figures out which words are wrong, and corrects them. And if there is too much wrong, it will simply delete the whole paragraph, kill the cell, and make you divide another one. But over time, these processes get less accurate. Your body is a human being, not a computer, so it does start making mistakes as you get older, and some of these errors slip through. [ON SCREEN: "Melanocytes die off"] Naturally, what you see is that the melanocytes sitting in and around the bulge of the hair follicle become less and less numerous. They start to die off, which means they are producing less melanin overall. So the color starts to fade. A lot of that is controlled by your genetics. Your genetics sort of determine how soon you are going to go gray, and your type of hair and your hair quality are largely determined by genetics too. But there are definitely lifestyle factors that can make it happen quicker, because anything that causes DNA damage will speed this up. A really big one is stress, but so are things like smoking, a bad diet, poor exercise, and poor sleep, all the general lifestyle factors that we doctors harp on about all of the time. These all lead to stress in the body, and that does translate, unfortunately, into DNA damage, which can bring this process on sooner than you would otherwise have expected. [ON SCREEN: "Your follicle bleaches itself"] But that still does not explain why hair goes fully gray. Why does all of the melanin disappear? Surely there should just be a gradual decline until you pass away, where it gets slowly grayer and grayer over time. You do sometimes see that. But there is another part of the process going on. Hair follicles have their own metabolism. They make and produce chemicals for themselves. One of the byproducts of metabolism in the hair follicle is a chemical called hydrogen peroxide, and some of you may recognise that name from other walks of life, particularly from hair bleach. Hydrogen peroxide is a waste product, a byproduct of metabolism, and it builds up in the hair follicle. Normally, your body has an enzyme called catalase, and it is the job of catalase to break down hydrogen peroxide and get rid of it. But again, all to do with aging, as you grow older you have less and less catalase, or the catalase you have is less and less effective. Which means, over time, that hydrogen peroxide starts to build up in the hair. [ON SCREEN: "Hydrogen peroxide destroys melanin"] I alluded to this a moment ago, but you could go out to the shops right now and buy yourself some hair bleach, not the bleach for cleaning the toilet, and you would notice that it is made out of hydrogen peroxide. That is what hydrogen peroxide does. It destroys melanin. So naturally, if you are building up hydrogen peroxide inside your hair follicles, it is going to literally bleach your hair, and therefore it goes gray, it goes white. Not only that, hydrogen peroxide also interferes with an enzyme called tyrosinase, and tyrosinase is very important for melanin production. So it is not just bleaching your hair by destroying melanin. It is also preventing further melanin from being produced. Overall you end up with a lack of pigment, and that means gray hair. We refer to this whole process, the hydrogen peroxide and everything around it, with a catch all term. We call it oxidative stress, which covers hydrogen peroxide and reactive oxygen species. So it is not just down to stress as to why you get gray hair. It is down to oxidative stress, which is not necessarily mental stress, although the two are related. [ON SCREEN: "Two sources of oxidative stress"] Speaking of oxidative stress, it is not only hydrogen peroxide at play here, because as you age you do not just go gray, you also lose hair, both men and women. For men, most of that is going to be down to testosterone, specifically DHT, dihydrotestosterone, which is the primary reason men lose their hair. We call that androgenetic alopecia. But hair does tend to thin out over time anyway. Even in the absence of DHT induced hair loss, you can have thinner hair in general as you age, a more diffuse hair loss. A lot of that is again down to the hair follicle itself being damaged. I am sure you will remember that the mitochondria is the powerhouse of the cell, and there are mitochondria in every single cell in our body, because that is how we make energy. Well, mitochondria release something called reactive oxygen species, which is also part of the oxidative stress pathway. So now you have two sources of oxidative stress. You have hydrogen peroxide building up in the hair follicle itself, and you have reactive oxygen species being produced by the mitochondria in the follicle and the cell. Put those two together, and you go gray. [ON SCREEN: "So how do we stop it?"] That is a fairly quick understanding of why hair goes gray. It is a little more complicated than that, but that gives you a good sense of what is going on. So if you are an enterprising, scientific person yourself, you would look at that and think, right, how do we stop this? The key thing is getting rid of hydrogen peroxide and getting rid of reactive oxygen species. And you would be one hundred percent correct. That is exactly how we try to stop it and how we try to reverse it. We have something called antioxidants. The reason they are called antioxidants is because they combat oxidative stress. That is the whole role of an antioxidant. Our body makes quite a few of them, but there are many more you can get from outside as well. There are so many fruits, plants, chemicals and compounds that have really high antioxidant properties. I am sure you have seen it plastered all over advertising, all over different products and different fruits. High in antioxidants. Well, this is what they are talking about. Antioxidants do a lot to combat oxidative stress, and oxidative stress is one of the main factors that drives the appearance of aging. [ON SCREEN: "What raises your oxidative stress"] Let us have a quick chat about what determines how much oxidative stress your body goes through. A large part of it, and a part we cannot really change, is genetics. If your mum and your dad were both gray by the time they were thirty, chances are you are probably going to be gray by the time you are thirty as well. There is not much you can do about that. That is genetics, and we cannot really change that at the moment. But there are other sources, and those are lifestyle factors. A really common one that people already know causes hair loss is stress. And here I am not just talking about oxidative stress, I am talking about mental stress, life stress, emotional stress. Chronic stress or a high pressure environment does lead to increased levels of oxidative stress. It does lead to you losing further hair and going gray a lot earlier, because it raises your cortisol levels, the stress hormone, and cortisol has been shown to be involved in processes like this. So from a lifestyle point of view, what you can do is manage your stress levels and manage your pressure. Make sure you are doing your hobbies, make sure you are eating well, sleeping well, all that sort of thing, in order to maintain a good healthy state so that you do not build up too much stress. [ON SCREEN: "Check these 5 nutrients"] Another big factor is nutrient deficiencies. The most common culprits are going to be B12, iron, zinc, folic acid, and copper. These are the deficiencies we usually find when someone is trying to investigate why their hair has gone gray. So, speaking of antioxidants. As I mentioned earlier, antioxidants combat oxidative stress, and we have shown that oxidative stress is the primary factor that leads to graying of the hair. So antioxidants are a good thing. How do we get them? Stay until the end here, because I am going to give you the number one master antioxidant, the one with the highest level of antioxidant activity, the one you probably want to take over all the others. But first, let me list out some of the common sources you can use to get a good load of antioxidants into your system. There is really no upper limit here. Antioxidants are great, they are always going to be great, so get as much as you can. Just be mindful of the other things that come packaged along with them. [ON SCREEN: "Food sources: vitamin C, vitamin E"] Let us talk about food sources. Two of the great antioxidants we have are vitamin C and vitamin E. You will find these in a lot of citrus fruit, in other fruits like acai berries, strawberries and raspberries, and you will find them in nuts and in seeds as well. So that is where your vitamin C and your vitamin E come from. As I said, watch the packaging. You do not want to consume fifty thousand limes or lemons a day, because you are going to have other problems from everything else that comes in those limes and lemons. Ideally you want enough to get a good level of vitamin C without going too far. You cannot just eat all the food in the world. [ON SCREEN: "Polyphenols + flavonoids"] Some more good sources of antioxidant activity are polyphenols and flavonoids. Where do you find polyphenols and flavonoids? They are very abundant in things like green tea, dark chocolate, and red grapes. And there is an easy hack here. Colorful vegetables. Most vegetables that have weird and wild colors, like yellow peppers, green peppers, red peppers, and fruits like pomegranate and dragon fruit, tend to be high in antioxidants. That is not one hundred percent true in every single case, but it tends to be fairly accurate. The more colorful the fruit, the more colorful the vegetable, the higher the antioxidant level is likely to be. Now let us talk about certain minerals, because we get minerals from our food as well. The two most important antioxidant minerals are going to be selenium and zinc. These minerals are important because they directly support what I referred to earlier as our number one antioxidant. [ON SCREEN: "The master antioxidant"] And that is glutathione. Glutathione, also written as GSH, is our body's preferred antioxidant. It is the one our body makes for itself in order to carry out its antioxidant activity, so it is probably the most effective form of antioxidant that you can take. Typically you cannot really get extra glutathione, because your body makes it by itself. What you can do is support your body's production of it. This is where selenium and zinc come in, because they directly support the enzymes that end up producing glutathione. That is why they are very, very good. But you can also take glutathione by itself. You can take it as a tablet, but far more effective is taking it as an infusion, mixed into fluid and delivered directly into the veins through a fluid drip, a vitamin drip of some sort. This way of taking glutathione is excellent. And the higher your levels of glutathione, the more antioxidant activity you will notice, which can stop and reverse graying of the hair, and can actually lighten your skin in certain cases as well. [ON SCREEN: "Liposomal or nothing"] Just as a quick aside on the current state of the market. You will find glutathione tablets all over the place, and the majority of those are not going to work. The reason is that glutathione gets destroyed by stomach acid really, really easily. So unless that glutathione is protected in some way, it is not going to make it past the stomach acid, and therefore it is not going to be absorbed into your system. So if you insist on taking an oral tablet or a supplement that contains glutathione, what you want to look for is a specific form called liposomal glutathione. This way, a large part of the glutathione does make it past the stomach acid, it can be absorbed, and you can do it that way. Just be careful. Do not buy any old glutathione. Get liposomal glutathione. [ON SCREEN: "Under investigation"] Let us talk about a few antioxidants that are currently under investigation. People have heard of these, you see them on YouTube, you see them on TikTok, and nobody is really giving much information on what exactly they are. When I say under investigation, I mean they show good promise, but has it been scientifically proven to work in all cases, and has the exact level of effect been determined? Not yet. That is why they are under investigation. The first group is a class of compounds called catalase boosters. Remember, catalase is the enzyme that gets rid of hydrogen peroxide in the hair follicle. Catalase boosters do exactly what it says on the tin. They boost the activity of catalase and therefore make it work more effectively. The next set are L-cysteine and glutathione precursors. A famous one a lot of people will know is N-acetylcysteine. That is a glutathione precursor, and precursor in this case means the previous product. N-acetylcysteine gets converted into glutathione, which is why it is a precursor. So NAC is a very common, very famous one, and there is another called L-cysteine. Both of these are precursors to glutathione and can help to increase glutathione activity, and therefore increase antioxidant activity. [ON SCREEN: "Resveratrol + curcumin"] The last group is certain plant extracts. One that is getting a lot of traction and a lot of investigation at the moment is resveratrol, which you primarily find in the skin of grapes, and another is curcumin, which you primarily find in turmeric. This is why a lot of people drink turmeric shots. It might also explain why people from India, Pakistan and that part of the world tend to have really good hair. They use a lot of turmeric in their cooking. And lastly, let us talk about lifestyle antioxidants. These are not physical sources of antioxidants. These are things you can do to boost your own body's production of antioxidants, regardless of supplements and any external factors. That is why I call them lifestyle antioxidants. [ON SCREEN: "Lifestyle antioxidants"] Number one is sleep. A good level of sleep, eight hours a day, every single day, good quality sleep, is going to increase your body's natural levels of antioxidant activity and glutathione. Another big one is avoiding stress, or rather managing your stress. It is okay to have stress. Remember, stress is not a bad thing. Stress is designed to move us, to push us to do something, because there is something we have to do. But some people have problems with managing it. They get way too much stress, they do not do anything to manage it or to sort out the underlying problem, and it builds and builds and builds. It causes all sorts of issues, including mental health issues. But in our case, what we are focusing on today is that it causes faster aging. It causes more oxidative stress. Therefore your hair goes gray, you lose hair, your skin looks more wrinkled, and you look worse overall with chronic high levels of stress. So managing your stress levels, however you wish to do so, is a very good thing for you to do. [ON SCREEN: "UV rays + pollution"] There are also things we can avoid. A really good one is avoiding too many UV rays. You do not want to be lying down in the sun turning into a roast chicken. You want to use UV blockers, otherwise known as sunscreen and sunblock, to avoid harmful UV rays, which everyone knows can lead to skin cancer. But of course UV also causes skin damage. You get faster wrinkling, more aged skin, and it can bring on graying of the hair a little bit earlier as well. And lastly, a more nebulous one, is pollution exposure. Depending on where you live there is not much you can do about this. Some places have a lot more pollution than others. But in general, air pollution is going to be a significant factor when it comes to oxidative stress in the body. [ON SCREEN: "For the science nerds"] For the more scientifically or chemically inclined among you, there are a few specific pathways you can go and research yourself that are involved in all of this. The first one is called the WNT beta-catenin pathway. This pathway is involved in activating hair follicle stem cells and activating hair follicle melanocytes. Then there is another one using something called MITF, which stands for microphthalmia associated transcription factor. This is the master gene, or the master signaling pathway, for melanin production in the entire body, so it is a very important genetic pathway to know about. The third area, also quite relevant, is the mitochondrial pathways and the oxidative stress pathways. There are two here to know about. One is called NRF2, which is a transcription factor for gene expression, and the other is PGC-1 alpha, which regulates the health of mitochondria in the body. I will not go into these pathways in too much detail. I think that is a whole separate video by itself, and it would be a very wordy, very scientific video. But just know that these scientific and gene pathways exist, and that there is active research going on to see whether we can affect them and therefore develop a cure for hair graying or hair loss. At the moment there is no real cure. This is all still scientific experimentation. [ON SCREEN: "Take home message"] So I hope that was useful in terms of why hair goes gray and what we can actually do about it. Let us wrap it up and give you some take home messages you can act on at home. The main causes are, number one, melanocyte stem cell exhaustion, which is the melanocytes dying off as you age. Number two, oxidative stress, which is hydrogen peroxide buildup plus reactive oxygen species production by the mitochondria. And number three, natural signaling pathway decline, those signaling pathways I mentioned earlier. Those three factors are really what causes graying of the hair. A lot of that is determined by genetics, so there is not much you can do there. Where you can actually influence things is with the oxidative stress and the melanocyte stem cell exhaustion parts, not really the signaling pathways. [ON SCREEN: "Get antioxidants in, reduce production"] On oxidative stress, to sum it up, you basically want to get loads of antioxidants into you, and you want to reduce your production of reactive oxygen species and hydrogen peroxide. Getting antioxidants in is going to be primarily food sources, plus glutathione as a major antioxidant that you can take externally. Reducing production is about looking after yourself, to put it simply. Proper sleep, proper food, proper diet, proper exercise, drinking enough water per day, and lowering your stress levels. That last one is the most important. Also, address deficiencies early. Check your nutrient levels and pay particular attention to B12, iron, folic acid, zinc, and copper, as these are the likely deficiencies that lead to early graying. If you are low in any of those, get to work on replacing them, either through diet or through supplements, in order to bring them back up. [ON SCREEN: "Be gentle with your hair"] And lastly, something I did not mention earlier. Be gentle with your hair. Do not use harsh chemicals, do not bleach it, do not keep dying it different colors. All of that stresses out the hair follicle and leads to premature graying and premature damage. Try not to use too many hair products either. A little bit of wax or hairspray is absolutely fine, but do not whack it in in huge amounts every single day. Overall, keep your scalp healthy and your hair clean. Increased blood flow to the scalp does help with hair growth. So keep your scalp clean, do not let it get dry, make sure it is moisturised well, and use something like Nizoral, which is an anti-fungal shampoo, once or twice a week, pretty much forever. That goes a long way to reducing scalp inflammation and giving you healthy hair overall. The script [ON SCREEN: "Exosomes: hype or real?"] Exosomes are being hyped as the next big thing when it comes to hair growth. So today, let us go through exactly what they are. Exosomes have become a prominent focus in hair restoration research, because they have been shown to have pretty good tissue signaling benefits. More on that a little bit later. There are multiple reviews, and I am talking academic reviews from 2025, this year, that classify them as a next wave biologic, as one step above PRP, and on a similar level to stem cell therapy. In fact, exosome therapy shares a lot of similarities with stem cell therapy for the condition of alopecia. Today I will explain what exosomes actually are, what the data really says, what type of person will benefit from this, how it compares to PRP, and at the end I will give you a five question checklist to protect your scalp and your wallet before you pay for exosome therapy. I am also going to be performing the treatment on myself, so you will have visual evidence in this video of exactly how it is done and how painless the process can be. [ON SCREEN: "What is an exosome?"] So what exactly are exosomes? Think of them as little biological messengers. They are small parcels, or vesicles, released by a lot of different cells within the body, cells like fibroblasts and stem cells, and they can do a whole host of different things. Scientifically, exosomes are classified as extracellular vesicles. They measure between 30 and 150 nanometres, so they are really, really tiny things within your body. The next question is what is inside these parcels. Inside them there is a rich cargo of proteins, lipids, messenger RNA, microRNA, and a huge amount of growth factors and healing factors. An easy way to think about it is to think of these as instructions that allow cells to talk to each other. They give cells instructions on what to do. When one cell releases exosomes, another cell can absorb them, read what is inside, and treat it like an instruction booklet telling that cell what to do next. This process is extremely important when it comes to healing and growth. [ON SCREEN: "The wound healing evidence"] Several studies have demonstrated just how powerful this communication can be. There was a recent review published in 2024 in a journal called Frontiers in Cell and Developmental Biology that showed clear evidence that, in wound healing models, so a lab scenario where a wound is healing, exosomes can stimulate angiogenesis, which is the formation and production of new blood vessels. They can guide fibroblast migration, so they lay down instructions on where to put extra collagen and where to build the structural component of the skin or tissue. And they can promote collagen remodelling, so not only laying down new collagen but also remodelling existing collagen into a stronger form. Every one of those steps is essential when it comes to healing wounds and growing tissue. So if exosomes can orchestrate that level of regeneration in skin and soft tissue, then researchers started asking a very important question. Can we deliver that same regeneration directly to struggling hair follicles, and if we do, will they grow? Let us have a look. [ON SCREEN: "Under the microscope"] What is actually happening under a microscope? Let us say you have gone somewhere and they are injecting exosomes into your scalp. Once that is done, those exosomes bind to specific proteins on, around, and inside the hair follicles, particularly the dermal papilla cells and the outer sheath of the root of the hair follicle. Once they are attached, these vesicles release the cargo we talked about earlier, and that cargo acts as molecular instructions, telling the hair to regenerate, telling the hair to grow, telling it to get denser and more numerous. For the more scientifically aligned among you, there was a review recently that established that exosomes work on two major pathways when it comes to hair. Firstly they upregulate the WNT beta-catenin pathway, and secondly something called the sonic hedgehog pathway, which is a great name for a pathway in my opinion. These are two of the same biological circuits that your hair follicles naturally use anyway to trigger what we call the anagen phase, the growth phase of the hair follicle. [ON SCREEN: "Longer anagen phase"] Let us look at some other experiments. These are animal experiments, so there is not too much data you can derive from them and apply directly to humans, but it is still useful data for us to know. What exosomes have shown in animal studies is that they prolong the anagen phase, they delay the catagen phase, and they can assist or accelerate the transition from telogen back into anagen. All of that put together means the hair follicle spends a lot more time in the anagen phase, the growth phase, and that translates directly into increased hair growth, denser hair, and more numerous hair follicles. Now we have gone through the science of how it works. So what does the human data actually show? The short answer is that it is still early data, but it is very promising. [ON SCREEN: "Pilot trial, South Korea 2023"] The first pilot trial we had came out of South Korea in 2023. A pilot trial is a sort of rangefinder trial, a quick, small study to see whether any results come from it. If it shows promise, then larger studies are usually built off the back of it. This trial involved 20 participants, all of whom were diagnosed with androgenetic alopecia. Over 12 weeks these participants received a series of injections of exosomes directly into the scalp. In terms of results, participants showed an average increase in hair density from 105 hairs per square centimetre to 123 hairs per square centimetre, as well as an average increase in the thickness of each hair from 58 micrometres to 64 micrometres. Both of these were statistically significant. What that means in the research world is that you can be fairly confident the difference was due to the variable being introduced, in this case the exosomes. It allows you to reject what we call the null hypothesis, the hypothesis that it does not do anything. What is even more encouraging is that in this particular study, no serious side effects were reported. Only some mild redness and mild tenderness at the injection sites, which is easily explained by the fact that you are using a sharp object to inject. [ON SCREEN: "Randomised control trial, 2025"] So that was the first set of data, from 2023. Very promising, but of course only 20 participants, nowhere near strong enough for us to make serious decisions about this. Then in 2025 there was another study, this time a randomised control trial, which is usually one of the highest levels of academic study you can perform. It means they take a lot of extra steps to remove confounders and remove conflicting information, and it was placebo controlled, which means they were testing the effect of the exosomes against the effect of basically nothing, or an alternative that does nothing. In this study, which ran for 16 weeks, participants who were all diagnosed with androgenetic alopecia tested a new formulation of exosome derived from an algae called ecklonia cava. So a plant based exosome, not human derived and not animal derived. Participants experienced significantly higher hair counts and greater shaft thickness, the thickness of the hair itself. Again this points to the fact that there is a genuine biological intervention going on here, and not just placebo. [ON SCREEN: "Systematic review, 250+ patients"] Let us zoom out a little further and look at one more study. In this case it was a systematic review done in 2025. A systematic review is a study that looks at other studies. It pulls together all the different randomised control trials and other work, goes through them, comments on the strength and validity of each one, then pulls all the data together and compares it as a whole. That systematic review pulled 11 human studies. Two randomised control trials, which is the strongest level of evidence, three prospective cohort studies, and several retrospective analyses and case reports. They found consistent positive trends across more than 250 patients. The reported average increase in hair count was between 9.5 and 35 new hairs per square centimetre, with hair thickness increasing by up to 13 micrometres. And importantly, no serious adverse effects were reported across any of these studies. These results suggest that exosome therapy so far is both safe and biologically active. But let us be honest, the human study data is still not numerous enough and not robust enough to make really serious decisions on. [ON SCREEN: "PRP vs exosomes: the source"] Let us talk about PRP versus exosomes. First, the source. Let us do a quick recap. What is PRP, how does it work, and how strong is the evidence for it? PRP is platelet rich plasma, and in order to make PRP we take it from your own blood. So if you go somewhere and get a PRP session done, the clinic or the practitioner will take some of your blood, usually between 16 and 20 millilitres or so. From there they centrifuge it, they spin it, and they extract the platelet rich plasma. Why do they want the platelet rich plasma? Because it is very high in growth factors like VEGF, which stands for vascular endothelial growth factor, PDGF, which is platelet derived growth factor, and IGF-1, which is insulin like growth factor one. All three of these are very strong growth factors that do a lot of different things in the body. You can inject them into the skin, you can inject them into joint cartilage, and you can inject them into the scalp around hair follicles, where they work and help the follicles to grow. [ON SCREEN: "Donor derived, not blood derived"] In contrast, exosomes are not derived from your own blood. They are donor derived. They are made from an external source, either animal based or plant based, and both work quite well. At our clinic, the formulation we use is of course plant derived exosomes, because of UK law, plus some extra medications, minerals and vitamins such as dutasteride, copper, zinc, tripeptides, biotin, and B complex vitamins. This cocktail is designed not just to promote regrowth but to stabilise DHT driven shedding, strengthen the existing hair that is already there, and improve scalp hydration and the microcirculation of the scalp, meaning blood flow in the scalp. Second, let us talk about the cargo, what is actually inside these things. I mentioned earlier that PRP contains a few growth factors. Exosomes also contain a lot of different growth factors. PRP delivers primarily growth factors derived from your own blood. Exosomes go a step further. Not only do they provide growth factors, they also provide things like messenger RNA and microRNA molecules, literally instructions telling your body and your skin what to do. That is why some researchers describe exosomes as the next wave of biological signaling, potentially more targeted than traditional PRP. [ON SCREEN: "Regulation: still investigational"] Third, regulation. PRP is well established in medicine and within medical aesthetics, and it is well tolerated. It has the go ahead from the FDA when prepared from the patient's own blood. Exosomes, however, remain a bit investigational. They are not yet FDA approved for hair restoration, and ongoing studies are still being done to clarify purity, dosage, injection technique, and clinical protocols. Fourth, let us talk about evidence. PRP has a wealth of evidence. It has a robust clinical track record showing and proving that it works when done with the correct technique. There are several meta analyses that show an increase in hair count from 35 to 105 hairs per square centimetre within three to six months, which is a pretty solid increase in the numbers. Exosomes, by contrast, are showing that even after just one or two sessions there can be between 9 and 35 new hairs per square centimetre and up to 13 micrometres increase in the thickness of the shaft. For one or two sessions, that is a great result. [ON SCREEN: "Safety profile"] And finally, safety and frequency. PRP is what we call autologous, which means it is derived from your own cells, your own blood, your own body, so reactions to PRP are very, very rare beyond mild discomfort and swelling at the injection site. Exosomes have also shown a very favourable safety profile, with similar effects such as mild swelling and mild discomfort at the injection site reported across hundreds of patients in multiple studies. So let us talk about who is actually a good candidate for exosome therapy. These tend to be people in the early to moderate stages of alopecia, whether androgenetic or not. That includes both male and female patients who have some thinning, some reduced density, or a widening part line. And importantly, it includes people who either cannot tolerate or are unwilling to try oral or topical DHT blockers like finasteride, or people who are certain they do not want to go down the surgical route and are looking for a good non-surgical option. [ON SCREEN: "Who it does not work for"] In several studies, the best responders tend to be people who still have existing hair follicles in the early to moderate stage. This is when hairs are still visible and can be seen under a microscope. It is not useful for people who have fully scarred or fibrosed scalps, which is very advanced hair loss, and it is not useful if there is another disease process causing the hair loss. In terms of results, this is not an overnight fix. Most patients see results within four to six months, and this typically requires multiple sessions. There was a case series comparison in the Journal of Dermis in 2025 that found early responders typically required only two sessions, spaced about three months apart, with a gradual increase in the number of hairs and the shaft thickness continuing for up to six months afterwards. But like all biologic treatments, outcomes are variable, because our canvas, the thing we are trying to get results in, is the human being, and human beings vary wildly in height, gender, race, genetics, and hair genetics. All of that matters, which means outcomes can be quite variable. Some patients notice a really clear, obvious difference in hair thickness, hair quality, and numbers of hair. Some people report only subtle changes, a little bit better but not obvious. That variability likely comes down to the factors I just mentioned. [ON SCREEN: "Not all exosomes are equal"] Here is another important point. Not all exosome products are identical. They do not all come from the same place. I mentioned earlier in this video that you can have animal derived and you can have plant derived, so that is already two very different sources. Differences in the source, in particle purity, and in dosage can all affect the outcome of exosome therapy for hair restoration. And finally, exosome therapy should not be seen as a standalone miracle cure. It is not going to do that for you. It works much better when combined with a proper hair restoration plan that includes DHT control, nutrition optimisation, scalp health maintenance, and regular follow ups to check on progress. So, in short, to summarise it all. If you are someone in the early to moderate stage of hair thinning, if you want a non-surgical regenerative option, and if you are ready to commit to a multi-month plan, then exosomes might be for you. [ON SCREEN: "Live demo"] Now let me show you the treatment being done live and in person, so you can see exactly what the process is like. What is going on in this part here is that first we are doing a little bit of micro needling, just to open up the skin, make a few small holes, and get some extra blood flow to the area. Then we apply the exosome solution that I explained before. Then we use the micro needles to insert that exosome solution to the exact right depth underneath the skin. This is much more accurate than doing it by hand, because we can set the device to an exact depth, whether that is 0.5 millimetres, 1 millimetre, 1.5 millimetres and so on. Essentially the deeper you go the better, but it does depend on someone's tolerance level, the sensitivity of their scalp, and how much they can handle. Typically, the first time someone does this they might only be able to tolerate a very shallow depth, but with repeated sessions their sensitivity reduces and they become much more able to handle deeper treatment. [ON SCREEN: "Controlled damage, on purpose"] Remember, you are not here just to mutilate the skin. You are targeting a certain level of damage. You are trying to cause damage, but on purpose, in order to stimulate certain processes, and then you add the exosomes on top in order to boost that healing potential and those growth and healing factors being released. So it is a controlled method of damage, admittedly, but damage with a good purpose. It is not too bad. It is a little bit of a pinchy pain. For me the level is about a two or three out of ten. You definitely feel it, a little bit pinchy. But of course I am doing this with no numbing cream whatsoever. You can use numbing cream such as Emla 5 percent, or other forms of numbing cream, to make it significantly more comfortable. For me it is fine, I can handle this, maybe a three or four out of ten. But if you are more sensitive, I would suggest using numbing cream, and we do use numbing cream as standard. [ON SCREEN: "Temper your expectations"] There are thousands upon thousands of substances, products, chemicals and medications that look good on paper but never make it past the clinical trial stage, because some problem gets found, or it is prohibitively expensive, or there are side effects, or it is simply not as effective as the science and the theory suggested it would be. So many of these problems turn up, and that is why we do clinical trials in the first place. Not only to see how effective something is, but also to find the drawbacks, because there are very few things in life with no drawbacks whatsoever. That is why I always caution people to temper their expectations. There are a lot of medications that never end up making it out for public use. That is why they are in the experimental research stage at this moment in time, and unfortunately a lot of hopeful products die in that stage because a problem gets found down the line. There is a saying we can apply to a lot of different parts of life. A bird in the hand is worth two in the bush. It is an old saying about hunting, that a bird you have actually caught, one that is in your hand, is worth more than two birds that are not with you yet and are still in the bush. In other words, look at what you can actually achieve rather than what you might maybe achieve. Same with these treatments. I would not discount existing medications, processes, procedures and techniques that we know work, in favour of potential future applications that may or may not ever make it to public use. [ON SCREEN: "5 questions before you pay"] Before you pay for exosomes, ask these five questions. Number one, what product are they using? Number two, what are the ingredients and the active compound? Number three, how are they administering it? Is it direct injection, is it through micro needling, or is it simply rubbed on top of the skin? Number four, what is the protocol, meaning the dosage, the number of sessions, and how far apart they are? And number five, what are the risks, the consent process, and the aftercare involved, and what happens if there is no response at six to twelve months later? If a clinic cannot answer these questions clearly, that might be your sign to hold off a little bit. [ON SCREEN: "You can combine them"] And just because I have gone through the differences between PRP and exosomes does not mean these things cannot be combined together. Because they work on slightly different pathways, the same broad mechanism but slightly different, and with different growth factors, for example PDGF which is platelet derived growth factor coming directly from platelets, mixing PRP and exosomes together can actually be very beneficial. It works through a completely different mechanism to finasteride. If you are thinking about hair loss, there are three major steps in the pathway that we can treat. The first one is reducing DHT overall, and that is what drugs like finasteride and dutasteride attempt to do. The second one is vasodilation and growth signaling, and that is what minoxidil does. Now the third pathway that has opened up is blocking androgen receptor activation, and that is what a new drug called clascoterone is attempting to do. [ON SCREEN: "Clascoterone"] The chemical name for it is cortexolone 17 alpha propionate. It is already being used as a treatment for acne, but for the last several years it has been heavily investigated for its role in hair growth, because it can block androgen receptor activation. Let us talk about the biggest barriers for people taking finasteride. Sexual dysfunction, mood disorders, lower energy, and fertility concerns. Theoretically, clascoterone could sidestep these concerns, because first, it is a topical formulation, and second, it does not reduce systemic DHT. [ON SCREEN: "This could work for women"] This means the drug could be used in females. At the moment females really only have one drug they can use, which is minoxidil, because they cannot use finasteride or dutasteride due to the systemic suppression of DHT. Well, clascoterone could be their answer. It has been in clinical trials since at least 2022, and it is in phase three, which means it is very close to attaining final approval for use. There are two major studies people are looking at, called scalp one and scalp two. Without going through the entire studies, scalp one showed an average of a 539 percent increase in hair count, or hair density, over placebo. However, scalp two showed a 168 percent increase in hair growth versus placebo. [ON SCREEN: "Why the huge gap?"] The main question that worries me here is why there is such a big difference in the outcomes. These two studies were designed to be quite similar in what they were testing, so they should be giving similar results. Either one of the studies was wrong in its scientific process, or the drug does not work as well as we think it does. Where I see a place for this drug in the future is as a complementary therapy. I do not think it is going to be strong enough to replace finasteride. [ON SCREEN: "After a hair transplant"] Now, how would this help patients who have had hair transplant surgery? Clascoterone is something they are going to be far more likely to say yes to compared with finasteride. And overall, after a hair transplant has been done, it is very important to have some form of DHT suppression to support the new hairs as they grow out, because they need that extra support to get nice results. It is a naturally occurring copper binding peptide. It has also been found in human saliva, urine, and wound fluid, and this is why some researchers think it is quite closely related to the aging process. [ON SCREEN: "Why people link it to hair"] But why do people think it is related to hair loss? Because it works in a few different ways. The first is fibroblast proliferation, then there is extracellular matrix remodelling, and then angiogenesis. We know that all of these pathways are involved with hair follicles as well. Hair follicles share many of the same pathways as skin regeneration, so directly improving wound healing or skin regeneration can indirectly improve hair follicle growth. [ON SCREEN: "It does not block DHT"] But there is a very important limitation. GHK-Cu does not block DHT. What that means is that it does not deal with the primary driver of androgenetic alopecia, which is why it cannot be compared to drugs like finasteride and dutasteride. The closest comparison is a drug called minoxidil. I have seen videos online where people have been using it for one month and they seem to have regrown an entire lion's mane of hair. I can almost certainly tell you that that is probably not true. There are no drugs, no peptides, nothing on the market whatsoever that I know of that can make that drastic an effect in as little as one month. Even hair transplants do not work that quickly. [ON SCREEN: "The honest verdict"] But it is not all doom and gloom, and I am not saying it does not work. As it stands, it is a very safe treatment and it is effective. It is just not a miracle cure, and it is not going to stop your hair loss. Keep your eyes posted for the next one.

Packed with over 200 different growth factors, proteins and anti inflammatory molecules, which makes it a touch better than PRP. And you can get results if PRP doesn't seem to be working for you. Why does this make a difference? Because when we use them specifically for hair follicles, they tend to reactivate those hair follicles so they grow at a much faster rate than they have currently been doing. They can actually reverse hair loss. By applying or injecting these exosomes into your scalp, you reactivate those hair follicles and you end up regrowing a lot more hair. Biotin, or vitamin B7, is one of our essential B vitamins, usually obtained through diet and food alone. But you can supplement this either in the form of tablets or by taking an injection. How does it work? Biotin does a lot of things in the body, but one of the main things it does is help produce keratin. Keratin is a protein which your hair, skin and nails are majority made of. But you may have seen this vitamin cropping up in a lot of different products like shampoos and multivitamins nowadays, and that's because biotin is generally considered a safe vitamin to consume, even in excess amounts. The reason for that is that it is water soluble, and even excess amounts typically get excreted into the urine. So if you're someone suffering from hair loss, then taking biotin could possibly help you if you have an undiagnosed biotin deficiency, and it is unlikely to harm you. So by the end of this video you'll know the difference between real science and marketing hype. Red light therapy uses low energy red and near infrared light, usually with a wavelength between 630 and 680 nanometres. That light penetrates the scalp and is absorbed by an enzyme inside your cells called cytochrome C oxidase, which lives within the mitochondria, which as we all know is the powerhouse of the cell. [ON SCREEN: "Charging a weak battery"] When this enzyme absorbs the light, it produces more ATP, which is the energy your cells use to function. It also reduces oxidative stress and inflammation, which are two major triggers for hair follicle miniaturisation. Think of it like charging a weak battery. That light energy recharges your follicle cells, giving them the fuel to stay longer in the growth phase, also known as the anagen phase, and it delays entry into the telogen phase, which is also known as the shedding phase. [ON SCREEN: "What the trials showed"] There was a double blind randomised control trial published in 2013. After 26 weeks, the treatment group had an average increase of 19 hairs per square centimetre compared to the placebo group. In 2014 there was a trial for women that showed almost identical results, around 20 new hairs per square centimetre after 24 weeks of consistent use. Then there was a meta analysis conducted in 2020. They concluded that low level laser therapy significantly improved both hair density and thickness in men and women with mild to moderate androgenetic alopecia, with no major side effects. [ON SCREEN: "Not every cap works"] Now let's be clear, that doesn't mean that every glowing cap on Amazon works. Many cheap versions that you can buy online don't use true medical grade lasers. They use LED bulbs instead. Think of it like the difference between a flashlight and a laser pointer. If the wavelength of the laser is not in that sweet spot, 630 to 680 nanometres for red light, or 810 to 850 nanometres for near infrared light, then it simply won't penetrate deep enough to reach the follicular bulb. Power density matters just as much as the wavelength. You need at least 4 to 6 milliwatts per square centimetre for it to trigger real biological change. [ON SCREEN: "How we actually use it"] It works best when it's part of a comprehensive plan, usually combined with PRP or exosome therapy. Typically we recommend three sessions a week, 15 to 20 minutes each, for at least 3 to 6 months before reassessment. Let's have a quick look. So I think there's been quite a bit of controversy around it, mainly because a lot of people seem to think it's much too straight. Well, I would agree. [ON SCREEN: "An inverted curve, not a straight line"] I think if you look at the pictures of when he initially went to Turkey and had his hairline drawn, they did draw a straight line. And as you can see in the full results now, after the transplant and having some time for it to grow, actually it doesn't even look straight anymore. It actually looks like an inverted curve, like a semicircle. So we can see at the front of his hairline it looks like it's higher than the sides of his hairline. And that is a much more female hairline rather than a male hairline. [ON SCREEN: "Male corners go up, female corners come down"] So the problem with doing it that way, when you create hairlines like that for a male, if I were to show you mine, the corners should go up. And that's natural, because that's the shape of your skull. That's the shape that natural hairlines grow. Now, I might be struggling a little bit, but your corners should go up as a circle. You can see on both of mine, like that. And it should come down in the middle. A female hairline is a bit different. Instead of going up in the corners, for females it usually comes down in the corners into their sideburn. [ON SCREEN: "The real failure was the consultation"] So the problem with this is I don't think the surgeon who performed this transplant has adequately explained to Mr. Bumstead exactly what the final result would look like. And in this case, in my opinion, I don't think he's had the best result. I do think it's a bit too high up in the middle, and it's a bit too low down in the sides, in the corners of the head, and that gives what some people are referring to as this alien hairline.

There is a small molecule. It's a topical molecule, which means you don't inject it, you don't have to swallow it, it's put onto the surface of your skin. And apparently within 7 days you can see some results and some increased hair growth. Now let's talk a little bit about how it works. The molecule in question is something called PP405, which is a research and experimental molecule at this stage. It's not marketed to the public yet. [ON SCREEN: "How it actually works"] So what does PP405 actually do? Well, it seems to target what's called the mitochondrial pyruvate carrier pathway, the MPC pathway, because this pathway ends up leading to increased levels of lactate dehydrogenase when it comes to hair follicle stem cells. Lactate dehydrogenase is pretty important for hair growth. So what PP405 does is increase LDH, which then ends up leading, through several pathways, to increased hair growth. There are some benefits to this, because it doesn't use the same pathway as DHT blockers like finasteride and dutasteride, which means it doesn't seem to have the same side effects such as low mood, low energy and potential issues with sexual function. [ON SCREEN: "The catch"] However, there are some drawbacks, because PP405 currently is a research chemical and it's only been used so far in rodents, mice, rats and so on. So it's not been used in humans, and for all we know there may be a complete difference when it comes to LDH levels in rat hair compared to human hair. But let's say everything goes well and we prove that it does work in humans. When is this likely to hit the market so that you can get your hands on it? We're looking somewhere near the end of 2028 to 2029, where we'll be able to pass all the phases of research and actually be launched into market.

Well, let's find out if this is actually because of the medication, or if it's because of something else. Rapid weight loss can actually trigger a condition called telogen effluvium. This is a form of temporary hair loss. It usually lasts around 2 to 3 months, and it's common to all causes of rapid weight loss. Another reason that these medications can end up leading to hair loss is that with them people usually achieve rapid weight loss, and the way they do that is by severely limiting or reducing their food intake, their calorie intake. The most important vitamins or minerals to worry about there are going to be things like iron, zinc, B12, biotin and vitamin D. As long as you're making sure that your vitamin and mineral intake is still good, then you should be fine. [ON SCREEN: "Less than 5% in the trials"] And lastly, again, rapid weight loss can cause some changes or shifts in your hormonal balances, and that can end up leading to hair loss. But don't worry, because it's not actually that common to have the side effect of hair loss while you're on these medications. The clinical trials have shown that less than 5 percent of people who are on these medications actually experience any form of hair fall. So what can you do? Slow down the rate of weight loss. Aim for about 1 to 1 and a half kilograms per week. Make sure your vitamin and mineral intake is still quite good. Don't be afraid of taking a multivitamin or something like that as a supplement. This will give you a good overview of your general metabolism and a lot of your major organs like the kidneys and the liver, as well as your risk for diabetes. And the third very important test to do is something called a thyroid function test. It's very important because low or high thyroid can cause issues and is very commonly missed, especially in women. [ON SCREEN: "Bonus add ons"] Here are some bonus add ons when it comes to blood tests, depending on your age and symptoms. Consider vitamin D. It's a very common deficiency found in the UK. Well, the reason we do blood tests, at least in medicine, is because it gives us a huge amount of information about your body that isn't readily apparent. For a lot of diseases associated with deficiencies of vitamins and minerals, or other problems, we can usually see them in the blood during testing way before symptoms ever actually appear. And even when symptoms have appeared, blood tests are a really useful way of us trying to figure out what exactly is causing that problem. Whether it's an infection, a deficiency of some vitamin or mineral, a hormonal problem or more, this can all be found out using blood tests. [ON SCREEN: "Thousands of tests. Which ones?"] Now, why should you come to me for blood tests? There are thousands of different blood tests that you can do that look at different things. Vitamin B12, we know it as B12, is also called cobalamin. It comes in several different forms like hydroxocobalamin and methylcobalamin, but essentially it's all cobalamin. Now, there are trace amounts you find in your food, but a lot of people are actually deficient in B12, and that's why sometimes we give B12 injections. The European data shows up to 60 percent of the older population may be deficient in B12, so it's quite a large chunk of the population. [ON SCREEN: "Enzyme one: making DNA"] What does it actually do? I think there are two major enzymes it helps to support. It works as a cofactor, which means it helps other enzymes do their job properly. So the first enzyme that B12 is very important for is something called methionine synthase. What does that enzyme do? B12 directly helps to produce more DNA, which is very important for your body.
